To comply with state and federal laws, certain genealogy requests require proper authorization before we can search, request, or obtain restricted documents (such as modern birth certificates, death certificates, or marriage records).

Please complete and submit our Authorization to Release Information & Request Vital Records Form if:

  • You are requesting restricted records

  • You want us to act on your behalf with a state or archive

  • A government office requires proof of permission

  • You are authorizing Nativa Genealogy to assist with document retrieval

This helps us legally obtain documents on your behalf and ensures your personal information is protected.

📝 How to Submit Your Completed Form

After completing and signing the form, please email it to:

Include any supporting documents required by your state, such as:

  • A copy of your government-issued ID

  • Proof of relationship (if needed)

  • Additional state-specific forms (we will tell you if required)


🔐 Why This Form Is Required

Some records are legally restricted and can only be accessed with:

  • Proper authorization

  • Proof of identity

  • Proof of relationship

  • A signed release form

  • A notarized form (in some cases)

This form allows us to ethically and legally assist you in obtaining these documents.

Once received, we will review your authorization within:

24–48 business hours

After approval, we will begin the research or record request process.
